Candace Tan, PharmD, BCACP
Dr. Candace Tan completed her pharmacist education and residency training at the University of Southern California, prior to joining the University of Hawai‘i at Hilo Daniel K. Inouye College of Pharmacy as an Assistant Professor of Pharmacy Practice.
She practices primarily in the outpatient care setting, working in interdisciplinary teams to provide medication therapy management for a wide variety of chronic disease states. Her research interests include developing pharmacist-driven outpatient clinical care programs and following practice-based outcomes at these sites. Her goal is to expand pharmacist care for underserved, minority populations across the Hawaiian Islands.
