Ms. Norine Wong, M.S.W., A.C.S.W., L.S.W., C.C.M

Norine Wong is a medical social worker/case manager with over 30 years of experience. Since 2012, she has been the VP of Operations and Business Development with FirstVitals Health and Wellness, Inc. where her current responsibilities include the implementation and sustainability of the Comprehensive Primary Care Plus (CPC+) program for participating physician practices by integrating team based care management. She believes in planning proactively for a life with choices and is integrating a program developed at Northwestern University Illinois called “PlanYourLifeSpan.” She is also a certified Diabetes Prevention Program (DPP) Lifestyle Coach, implementing a self-management program with a focus on prevention of microvascular diseases.

She is also a Medical Social Work staff member with the Pacific Islands Geriatric Education Center and UCERA.

She previously worked as: Clinical Director at Ho’okele Health Innovations; Executive Director of Community Case Management Corp, a private case management company she designed and managed; Director of Family Services at Shriners Hospital for Children; and medical social worker at Queen’s Medical Center.

Ms. Wong was born and raised in Hawai‘i. She enjoys travelling and exploring new places and cultures
